Online Services

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

Detailed Coded Record Access.

 

From 31 March 2016 all practices in England are required to offer online access to their record detailed information from the GP record, i.e. information held in coded form.

 What is meant by detailed coded record view? What will the patient be able to see?

The online detailed coded record refers to items in coded text, as opposed to ‘free text’. The patient will be able to see demographics, diagnosis and conditions, medications and latest issue date, as well as numerical results information.

Online Appointments and Prescription Requests with SystmOnline

You can book or cancel appointments, and request prescriptions online. To register please come to the practice with proof of ID (driving license/passport) and reception will be able to give you a User Name and Password.

Latest news 

Dear our PPG Members ,

Just would like to update you with latest news from CGMC .​

We would like to have face to face meeting but we will not be possible to do so probably up to mid of September 2017, because overtly we are now in the Holiday season.

Just to renew you up to-date on new things. We have been trying since you have raised the issue in 2015 to get a female doctor here, however you may be aware that:

·         One – there are small number of doctors year on year,

·         Two – they do not want salary positions, they do not want partnership, and they want to stay as a locum. 

We know that all patients would like this, however at the moment we will not be able to secure anyone on upon this basis. What we may be doing in future is trying to get a long term locum but we currently have done our best and we still cannot get one.

Our CQC report has not been favourable and has been disappointing. We are working hard to get out everything in place that has been raised.   We are in a nebula stage but we are hoping that soon everything will be improved.

 We are have two new staff members. Please make yourself known to them.

 We are still promoting OnLine booking service so that those patients who wish to make appointments can do so without needing to telephone the practice.

 Practice will not change booking system to fully automated system as some of the patients would rather speak to reception staff.  But will be very beneficial for you and us if you will use this system

 

 Note:

During the busy time if you are early for your appointment can check in using our self-check screen, but if you are late for 5 minutes  the screen will not allow you check in, therefore please notify one of the receptionists that you are here

Disability

Our premises are fully accessible to wheelchair patients and patients with restricted mobility.

Important: Staying registered

Your NHS Health Authority is in the process of confirming patients' address information. If you have received a letter from the NHS it is important that you respond to it. If the letter is not returned they may remove you from our practice list. If you received the letter and didn't respond, all you have to do is call the surgery and confirm your address with reception.

Family support service

These are FREE family-based, community interventions delivered by a Nutritionist and Physical Activity Leader and include:

  • Dietetic / Physical activity 1-2-1 service - parents with children aged 0-4 years
  • MEND Mum’s - post natal weight management programme for women with a baby up to 2 years 
  • MEND Mini – family programme for children aged 2-4 years
  • MEND 5-7 and MEND 7-13 family programmes
  • MEND Teens – healthy lifestyle programme for 13-18 years
